(Adult Liability Waiver/Release are required for any adult participating in an HSP event)

(This form should be used for all children under the age of 18 and requires both parent and child to sign.)

(A Medical Consent form is required for all children under the age of 18 that participate in an HSP event. The form must be on file prior to camp participation.)
